1. Login and click forms

2. Click donate to edit the donation form.

3. Click on the apply to field (scroll down if needed) Then click on the Edit Choices button on the left.

4. Click the + and - buttons to add and remove options as needed. You can Drag and drop the icons on the left to change the order of items.
